Integrity disruption is known as adverse modification (alteration).

Solving integrity in practice: Breach of information or data integrity is one of the risks that can affect any organization. Adverse change of data may not always be intentional to harm the organization. Breach of integrity can also result from a technical failure or force majeure (e.g. natural disaster). If the change in data is not detected, its effects can often be worse, because the organization operates on the basis of bad data. One of the measures is to audit all changes to data in order to monitor whether adverse changes in data occurred and by who they were performed.
